Insurance
and Medicare Information
The company from which
the purchase was made MUST submit Medicare claims.
After we have received your payment, we will submit
your Medicare form and you will be reimbursed directly
by Medicare.
Regardless of
when you had surgery, Medicare provides partial
reimbursement for breast forms and mastectomy bras.
Currently the allowable is
- 2 prostheses (1 silicone
& 1 leisure/swimwear) every other year and
- 4 mastectomy bras
per year.
Please note, each Regional
Medicare office might have a different allowable regarding
breast forms and mastectomy bras.
Medicare and most insurance
companies cover a percentage of pocketed bras and breast
forms. If you carry supplemental medical insurance,
please be sure to contact your insurance provider for
reimbursement. If you are not eligible for Medicare,
be sure to file a claim with your health insurance provider.
If you reside in Ohio, please let us know. Our Shoppes
might be a provider for your insurance company.
A Medicare information
sheet will be sent with your order. If you are eligible
for Medicare, fill it out and return it to us with your
doctor's prescription, dated on or before the date of
your purchase. Inquiries about reimbursement should
be made through your Medicare office.
Please allow 8-12 weeks for reimbursement.
Ida-Joy Shoppes cannot
